Palladium in poor condition

Good Morning,
I check the Palladium starter source after a complaint and sure 
enough it's in poor condition. There isn't a hole, but metal sticking 
out from the bottom which won't allow the
source to stay cool. Another  Pd starter source had been on ordered 
and will arrive this week.
